Those trial figures are an approximation and we would rather say so than present them as exact: the 6-month trial is half of a twelve-month year, so on the annual plan it offsets roughly half the first year's fee depending on when you start. The precise per-year figures.
Paying monthly across four years
The monthly plan costs 89.88 dollars a year, so four years is 359.52 dollars — about 83.52 dollars more than the annual plan over the same period.
That is the compounding version of the 20.88 dollar annual difference, and it is the strongest argument for the annual plan — provided you will still be eligible for the year you have prepaid. Monthly or annual.
The four-year limit is not a coincidence
Amazon's end condition on the student route is four years or the end of your studies, whichever comes first. So a standard four-year degree and the maximum run of the discount are the same length by design.

What happens in year five
The membership converts to regular Prime at $14.99 a month or $139 a year. Nothing about the benefits changes; the price roughly doubles. Amazon says it will email several weeks beforehand if it believes you are no longer eligible.
That conversion is the moment to make a real decision rather than let a renewal happen. Plenty of people who were right to hold the membership at $69 are wrong to hold it at $139. The break-even against no membership at all.

Putting the number in proportion
Around 241.5 dollars across a degree is not a large number against tuition, and it is a meaningful one against a monthly budget. Both of those are true and the second is the one that matters day to day.
The useful test is not "can I afford $7.49 a month" — almost anyone can — but "does this save me more than 5.75 dollars a month". That is a real question with a real answer, and for a lot of students the answer is no. Who should skip it.
